  4. Upon my disability, the designated guardian shall have the following
  5. authority:

  8. a) residential custody of the minor child(ren);
  9.  
  10.  
  11. b) to approve medical treatment of any kind or type or
  12. to disapprove the same within the bounds of the law;
  13.  
  14.  
  15. c) to designate schooling for the minor children, and
  16. access to any and all of their educational records;
  17.  
  18.  
  19. d) to generally act in loco parentis.
  20.

  22. In the event that I am the custodian of any property for
  23. the minor children under the Uniform Transfer to Minor's
  24. Act, or the Uniform Gifts to Minors Act or similar statute, 
  25. I designate the guardian or successor guardian to act as
  26. custodian for all such custodial property.

  29. In the event that formal legal proceedings are commenced to
  30. establish a guardian for the child, it is my desire that
  31. the guardians mentioned herein have priority in appointment.

  34. The failure to list an individual as a guardian or successor
  35. guardian is intentional.
